To prevent recurrence of the problem, you need a root-cause analysis. Getting rid of the root-cause will eliminate a similar problem from occurring again. Also dealing with the root-cause will assist in the planning to prevent something from happening. For example, in an engineering setup, a problem to the plant might just be caused by incompetent operations staff. This gives the management information on how to prevent this problem. Management, in this case, will need to make sure their staff is well-trained and competent enough to handle the given plant.
At this stage, the team members come up with different ideas on how to solve the problem. Various ideas are generated, and at the end of it all, the best solution will have to be selected from the suggested solutions.

As you attempt to find a solution, it is important to look at the problem from different points of view. This will help you understand the problem and solve it effectively.
